What is REBOA?

REBOA (Resuscitative Endovascular Balloon Occlusion of the Aorta) is a life-saving technique used to temporarily occlude the aorta by endovascular means. A balloon catheter is introduced through the common femoral artery and inflated in the aorta, providing an internal clamp. This provides a bridge to get the patient to definitive care.

REBOA Zones

There are two different zones where the aorta is typically occluded for REBOA, depending on the extent of support required by the patient.

Zone 1:

Proximal Aortic Occlusion

Zone 1 extends from the left subclavian artery to the celiac trunk. Occlusion in Zone 1 provides the most significant increase to a patient’s blood pressure and is therefore typically used when significant circulatory support is required.

Zone 3:

Infrarenal Aortic Occlusion

Zone 3 extends from the renal arteries to the aortic bifurcation. Occlusion in Zone 3 provides control with less of an increase in blood pressure than Zone 1. However, this allows for continued blood flow to the abdominal organs and reduces the risk of ischemia reperfusion injury.

No REBOA Zone

Pre-hospital REBOA

Pre-hospital REBOA is becoming more prevalent in certain settings. Use of REBOA in pre-hospital settings requires critical preparation, training, and protocols to facilitate its use by emergency personnel. As this practice grows, the benefits of the COBRA-OS® can provide advantages that are most suitable for the pre-hospital environment.
